Community Health Worker (CHW) 1/2 time
Mothers & Babies Perinatal Network is looking for a part-time (20 hrs/wk) Community Health Worker with experience in providing services to a high need population. Eligible candidates are friendly outgoing individuals with experience connecting individuals to community support services and providing assistance with application for these services as necessary. The ideal candidate is comfortable meeting new people, is able to provide home visitation, is organized and has excellent verbal and written communication skills. This position requires a valid driver's license and reliable transportation.
CHW's may also provide home visitation services in instances where a client is not eligible for other home visiting programs within the community. Candidates must be comfortable with provision of information about community supports including: health insurance (including FPBP), primary care and/or prenatal care services, family planning services and other needed community services such as WIC, substance abuse, domestic violence, mental health, etc. CHW's will receive all necessary training regarding available services and will work closely with the CHW Coordinator and Perinatal Health Director to develop individual plans for families.
The CHW is an hourly employee with a pay rate of: $11-$13/hr dependent upon experience. Our goal is to hire the ideal candidate as soon as possible. Candidates should submit detailed resume, plus salary history and expectation to:

5 Full Time Americorps Position Available within Health Insurance Program
Health Access Specialist's will work closely with staff in our
Health Insurance Program as the boots on the ground throughout a 5
county region of south central New York State (Broome, Chemung,
Delaware, Otsego and Tioga). They will plan an integral role in
building individual and community capacity in urban and rural
communities linking people to needed health insurance/access
knowledge through a wide range of activities such as outreach,
community and school based education and informal counseling.
They will provide accurate and up to date information about how to
access coverage through the New York State marketplace and direct
consumers to the in-person assistors (Navigators) who can provide
health insurance enrollment assistance. Successful candidate will
